Nicomide^® Rebate

Please download and print one copy of the accompanying PDF rebate certificate and follow these instructions to obtain your Nicomide rebate.

1.Purchase a bottle of Nicomide tablets.

2.Completely fill in the mail-in certificate.

3.Attach the original pharmacy prescription receipt with the product name and price listed.  Cash-register receipts will not be accepted.

4.Make copies of all documentation for your records.

5.Mail the completed certificate with original pharmacy prescription receipt to:
